Care Management is a vital connection linking the patient, payor and all treating clinicians. Care Managers act as advocates, communicators and catalysts for the patient, the provider and the payor. The Need for Medical Care Management

Medical Case Management has become a crucial feature of risk management programs and it is necessary for controlling the escalating costs of serious illnesses and chronic diseases. Care Management assures that the medical services the client is paying for are essential, effective and being delivered in the most appropriate, coordinated and cost-effective manner available.

 

Scope of Services

As part of our Medical Care Management Program we include but are not limited to the following;

  • Assess the needs of the patient and family

  • Identify resources

  • Coordinate care

  • Negotiate ancillary services

  • Assess appropriateness and medical necessity of care

  • Coordinate discharge planning

  • Assist the patient in understanding his/her condition

  • Assist the patient/family in appropriate medical care decisions

  • Represent the client as a qualified and objective third party

  • Assess quality of care

  • Provide network assistance

  • Provide Physician Advisor assistance

  • Provide nationwide coverage

  • Perform on-site visits upon request

Cost

Strategic Health offers either an hourly charge, based on 1/10th of hour increments or a PE/PM arrangement for pricing. The Care Management services are available twenty-four hours per day, three hundred and sixty-five days a year.

 

Commitment

Strategic Health's Care Managers are highly qualified nurses with backgrounds in intensive care, emergency room, oncology and transplants, and the majority are certified in Care Management. Strategic Health maintains flexibility to meet the needs of the clients and is committed to ensure the care being provided is high quality yet cost-effective.
